There is a lot of information in the form. Fill in only as much information as you know or have on hand and that you believe to be correct. The more information you are able to fill in the more complete and accurate your family record will be.  The information you provide will be entered into a computer program and used to research and document your family. When you are finished click the Submit button at the bottom of the page.  Only members and branches of the families that are the subject of this website should use this form since the data will be automatically emailed to a researcher for these families.  Click the Print button to save a copy of your data before clicking the Submit button.
